How to obtain a copy of a medical record

Please print, complete and sign the request form below to obtain copies of your child’s medical records.

Mail or fax your signed request form to the address or fax number located at the bottom of the authorization form. You may also email the form to roi@childrenscolorado.org or leave the form with the Release of Information department during business hours.

Note: Release of Information cannot review, discuss or interpret medical records. Please direct these questions to your healthcare provider.

Important notes about your child’s medical record request

Requests for “complete records” (including progress notes, consent forms, flow sheets, etc.) can delay processing, become very costly and is usually much more information than you need. If you need help deciding what to request, please contact Release of Information.

  • Personal representatives will not be provided access to a minor’s records if the minor was legally able to consent for treatment, unless the minor authorizes access to this information.
  • Access may be denied if the information is not part of the designated record set or if it is determined the access could endanger or harm the patient or others.
  • Patient signature is required if the patient is 13 years of age or older and reproductive health (including pregnancy and sexually transmitted disease), HIV/AIDS or drug/alcohol treatment information is being requested.

Patient signature is required if the patient is 15 years of age or older, and behavioral health or psychiatric care information is being requested.

Receiving your child’s medical records

Processing medical record requests will take up to 10 business days from date of receipt. Records will be released and delivered in the method indicated on the authorization form. Occasionally delays occur and we will attempt to contact you if extra time is needed to process your request. Please bring a photo ID if you choose to pick up the medical records in person.

Payment for medical records

Colorado State Law authorizes Children’s Colorado to charge for copying medical records. Charges, if any, are due at the time you receive your records. Release of Information accepts check and credit card payments only. If you have a medical record invoice, you can pay online.
